Hemin powder is a metal porphyrin compound with relatively simple structure, formed by the complexation of a molecule of ferrous and protoporphyrin. Exogenously administered heme inhibits the activity of aminolevulinic acid synthase (ALA synthase) in the liver. ALA synthase is the first rate-limiting enzyme in the porphyrin synthesis pathway. By inhibiting this enzyme, heme reduces the production of toxic porphyrin precursors, thereby alleviating the symptoms of acute porphyria attacks.
